You can test the mouthpiece by recording yourself and playing the recording. The top mic can be tasted by recording a video and listening to the sound quality.
They're two mics. The first one is located on the left of the lightning connector and the other is near the front camera.
This can be due to software hitches, some apps limiting mic use, some settings inhibiting mic use or hardware defects.
Of course, there was a recall for devices manufactured between October 2020 to April 2021 that had sound emissions when receiving or making calls.
